II-IV: "The original music in the Witches Scene, in Middleton's comedy of the Witch, from a M.S. of that age"--apparently copied from John Stafford Smith's Musica Antiqua (1812), p. 48-49; the original manuscript source is Drexel 4175, the attributed composer is Robert Johnson (information from John P. Cutts, "'Songs Vnto the Violl and Lute': Drexel Ms. 4175," Musica Disciplina 16 (1962), p. 73-92).
  3. Preface dated 31 Jan. 1822 has different publisher's address (133 New Bond Street) to that on title page (140 New Bond Street); Birchall was publishing at 140 New Bond Street begining in 1824 (information from Humphries, Music publishing in the British isles, 1970).
