Identification

In order to complete registration for an ACCESS card, photographic identification must be presented at an ACCESS card desk.

One piece of ID with photograph, name, and address is acceptable, such as:
· Driver’s license
· Government issued ID
· Homeless agency issued ID

A combination of 2 pieces of ID can be used if necessary; One a photo ID with name and photo and one proof of address with name and address.

Acceptable photo ID include:
· Student ID
· Employer ID
· Credit card with photo
· Passport

Proof of name and address ID include:
· Voter registration card
· Utility bill from the last 3 months
· Rent/maintenance receipt from the last 3 months
· Non photo driver’s license
