Get the Facts about Affordable Care Act
Stop by your local library to get the facts about Affordable Care Act (also known as Obamacare) and learn how you can enroll into a health care plan.
This information and enrollment session is designed to provide consumers and small business owners with information about the Affordable Care Act, the new health insurance marketplace and the enrollment process and get assistance to enroll into a health care plan with a trained Navigator.
1. Social Security Numbers (or document number for legal immigrants)
2. Birth dates of family members
3. Employer and income information for every member of your household who needs coverage (for example, from pay stubs or W-2 forms – Wage and Tax Statements)
4. Policy numbers for any current health insurance plans covering members of your household.
